So I use the consent form from my hospital for taking medical images. I explain cc licenses to patients and explain that I plan to put the image on Wikipedia and add those details to the consent form. I than keep a copy of the consent in my office records and put one in the patients chart at the hospital. Commons does not need the consent form but your College of Physicians and Surgeons may if anyone ask through them. I would also recommend that you remove the EXIF data for the data of the image and for the location. Commons does not make it easy after the fact unfortunately. I can send you a copy of the consent I use if you want. PLOS also has a pt image consent form they use.

Je vais lui demander le formulaire qu'il utilise. Effectivement, il va bien plus loin que ce qui simplement demandé par Commons.

Voici également le formulaire de PLOS, qui est plutôt bien monté car:

• il fait mention de l'usage primaire de l'images (la publication dans un article scientifique);
• il évoque aussi les conséquences possibles du partage de l'image sous la licence CC-BY, c'est à dire son usage commercial et/ou à d'autres fins.
